Raritan Family Health Care is recognized as a Level 3 Patient-Centered Medical Home by The National Committee for Quality Assurance (NCQA). 

We use a team approach to providing primary health care, including your health care provider, others who support you, and — most importantly — you. We use technology such as our patient portal to communicate and coordinate your care and provide the best possible outcomes for you and your family. Through Medical Nutrition Therapy paired with motivational interviewing techniques, Elisa has helped individuals sustain positive behavior changes to create a healthy relationship with food for optimal health.

Jackie likes to provide a non-judgmental, safe, warm, and accepting environment.  Her goal is to help you live a most fulfilling life.  Her areas of expertise include anxiety, depression, personality disorders, life transitions, self-esteem, relationship/family issues, substance abuse, and trauma. She is trained in Cognitive-Behavioral Therapy, Dialectical Behavior Therapy, Trauma-Informed Care, ARC GROW, and Motivational Interviewing. She has worked in public and private sector with youth, adolescents, adults, families, couples, partial hospitalization, in-home, and nursing homes.
